Native American patients can enroll in dental services. Patient must bring in proof of income to qualify for sliding fee discount for dental services, status card, drivers license or state identification. At the time of enrollment a $20.00 registration fee is collected, dental application is processed by front desk staff and faxed to dental office. The patient is given a brochure with the dental address and services and they are responsible for making their appointment. Keep in mind this covers preventative services, such as, oral evaluation, x-rays, cleanings, topical flouride application, tooth sealant, 4 fillings, and tooth extraction without oral surgery. Any other work is the patient's responsibility.
